Fertility Assessment
Any couple who have been trying for a baby for more than 12 months (or more than 6 months if over 35 years old) should have a fertility assessment. Note that this does not necessarily mean that fertility treatment is required, just a check that all of the right factors needed to allow natural conception are working well.
First of all, a careful history needs to be taken from both partners, or from the one patient (if single).
The consultation will concentrate on the underlying patient factors, previous reproductive history, past history and symptoms indicative of reproductive conditions, such as pelvic pain for endometriosis, or irregular periods for Polycystic Ovarian Syndrome. If patients have presented for a second opinion, extensive detail of past fertility treatment is very helpful.

Personal and Individualised Treatment Plan
The basic factors which should be the focus of investigation, so that a personal and individualised treatment plan can be made include:

Ovarian / egg reserve
with a blood test, such as an AMH level (Anti Mullerian Hormone), and / or a pelvic ultrasound (Basal antral Follicle Count).

Ovulation
with history of regular periods, and sometimes a urine test or blood test.

Tubal patency
with a careful consideration based on patient history and symptoms as to whether diagnostic imaging with dye (Hysterosalpingogram, or HyCoSy specialised ultrasound), or a short surgical procedure to insert a telescope camera to investigate the pelvic and tubal anatomy (Diagnostic laparoscopy with dye studies) is the best method of assessment.

Semen analysis
testing for the concentration, motility, and morphology (or shape of the sperm).
